You can bring a current check stub, current tax return, or an assistance letter with a dollar amount (Food Stamp Letter, SoonerCare (Medicaid) Eligibility Letter, Disability) are a few of the accepted items.

Generally, the clinic you are being referred to will contact the patient after receiving the information to schedule the appointment. In some instances, a member of our team will contact you with more information on scheduling.
